Activity Waiver & Photo Release

Activity Waiver: I waive, release, and discharge OCAC, its board, employees, instructors, volunteers, interns, and agents from all claims for injuries or damages related to program activities.

I agree to indemnify, hold harmless, and defend OCAC, its board, employees, instructors, volunteers, interns, and agents from any claims for injuries or damages arising from program activities.

In an emergency, I authorize OCAC staff to secure necessary medical treatment, and I will pay for all medical services.

Photo Release: OCAC may use your likeness in promotional media. Participation implies your consent.
